- Nature of your pain/symptoms
- How long you have had this?
- What have you done so far to help?
- Has anything been successful?
- Are you currently on medication?
- Have you had X-rays or MRI?
- If yes, do you know the results?
- What activities cause the pain/symptoms to worsen?
- A best time and phone number to be contacted
- Email or phone preference?
