Thank you for giving us the ability to help make a difference in your health and life. Our office has created this outreach tool so that prior to seeing us as a new patient, you can speak to the doctor, if desired.
There are two choices - email or phone. If you would like to leave Dr. Ambrose a descriptive email, you may do so by clicking the link just below in blue.
Otherwise, you may leave a message for the doctor at 750-2400,extension 4 upon initial connection to our auto attendant. It is very important that you leave as much information as you can pertaining to the following:
- The nature of your pain/symptoms
- How long you've had the symptoms
- What measures have you taken and doctors you have seen
- Current medications (type of meds, exact names not needed)
- Results of past X-rays, MRI or other imaging
- Activities which improve or worsen your symptoms
- A best time and phone number to be contacted
